Pathstones, a continuing care at home (CCAH) program, affords members more autonomy to make their own healthcare decisions, enjoy financial security and have peace of mind than long-term-care insurance plans, explains Cynthia Bonney, MBA, NHA, Executive Director of Pathstones by Phoebe. Unlike traditional continuing care retirement communities (CCRCs), Pathstones allows members to remain in the security of their own homes even as their healthcare needs change.
Pathstones’ emphasis on wellness and its coordination of services with members’ primary care providers and specialists dovetail with the outcomes-based goals of healthcare reform and make the program a valuable tool for physicians to help patients manage their health before urgent care needs arise.
“Wellness is our primary focus, and through monitoring, intervention and preventive programming, we provide the services that allow our members to age in place,” Bonney says. “The undivided attention our members receive may minimize the need for hospitalizations and limit the need for more expensive medical services later.”
Right from the Start
On a new member’s first day, a Pathstones wellness coordinator will partner with the member and their physicians to construct a personalized care plan that proactively helps to keep the member well and maintain their highest level of independence.
In addition, while Pathstones enables members to remain in their own homes, it promotes socialization by giving them access to amenities at Phoebe’s CCRCs, such as the indoor pool and Boardwalk at Phoebe Berks, and group nutrition and brain fitness classes at Phoebe Allentown.
“The combined approach of personalized preventive and wellness programming helps to promote a healthy, happy and independent lifestyle into retirement,” Bonney says.
Should members’ conditions change, wellness coordinators can arrange for additional services, including home maintenance, in-home nursing care, live-in support, medication management and physical therapy.
The program also reduces financial stressors related to increasing medical needs.
“People want a plan in place before times of medical crises to protect their assets,” Bonney says, “and to proactively determine the direction of their care so they do not burden loved ones.”
Members pay a one-time membership fee and monthly fees based on their age and plan selection. Fees are generally less than those of traditional CCRCs and cover future healthcare costs. If patients require additional services or transfer to a CCRC or nursing home, their fees do not change — inflation is the only factor affecting monthly rates.
Additionally, if members have long-term-care insurance plans, they can keep them — Pathstones enhances their coverage.
“We customize their Pathstones plan to fill gaps in their existing policy,” Bonney says. “We offer more comprehensive and value-based options than many long-term-care plans.”
Continuity of Care
The relationships cultivated between Pathstones’ members and their physicians are a critical component of the program’s success.
“Our members choose who cares for them,” Bonney says. “They can remain with the same primary care provider and specialists they’ve had for decades. Wellness coordinators work with physicians to provide an additional layer of healthcare assistance, for example, arranging transportation and coordinating medical appointments, and keeping the physician informed of any healthcare changes.”
